HIM Coordinator
Abstracts patient medical data from the medical record to appropriately complete the Meditech EHR abstract on each inpatient, Emergency Department and outpatient visits.
Accurately sequences codes according to UH DDS guidelines, basic coding principles and hospital specific coding guidelines. Assigns CPT-4 procedure codes to Same Day Procedure, Emergency Department and Special Procedure medical records while maintaining a 95-100% accuracy rate.
Validates the accuracy and sequencing of assigned ICD-10-CM and ICD-9-CM codes to assure the correct DRG/ARP DRGs are assigned, while maintaining a 95-100% accuracy rate.
Assigns appropriate E/M levels for professional services.
Conducts regular audits and coordinates ongoing monitoring of coding accuracy and documentation adequacy.
Reviews claim denials and rejections pertaining to coding issues and educates coding staff accordingly.
Educates physicians in clinical documentation improvement/lCD10.
Functions as acting HIM Director in the Director's absence to ensure the continued productivity and workflow of the HIM department. Other duties as assigned.
EDUCATION/LICENSURE/CERTIFICATION/REGISTRATION:
Certification Required: Registered Health Information Administrator (RHIA), Registered Health Information Technician (RHIT) or Certified Coding Specialist (CCS).
Thorough understanding of the effect of data quality on prospective payment and outpatient reimbursement. Solid understanding and command of the ICD9-CM and ICD10-CM classification systems and diagnosis related group (DRG) methodology. Previous coding experience in ICD9-CM, ICD10-CM, and CPT4/CHPCS required.
Other requirements: ICD10 Coding Experience; physician query process experience; chart audit experience.
